No. Procedure to Register Claims on Website
1 Launch Kshema Website: Kshema General Insurance | Crop Insurance Company
2 Choose from options available for Claim Intimation:
• Install Kshema App OR
• Continue with Web
3 Once redirected to the app/website, select Product Name and provide registered Mobile Number for 1st step verification
4 Once authenticated, follow the instructions as mentioned to fill the claims form and submit mandatory documents to complete the process of intimating a claim on the website/app.

List of Claim Documents required for Claim Processing

  1. Images of Crop Damage
  2. How to capture the photographs:
    1. While taking the geo-tagged photographs as per the direction mentioned column of above table, every corner photo should cover the adjoining two edges of the field.
    2. For the zoom level photographs need to ensure that the insured target crop should be visible to interpret crop stage, damage, and event occurrence.
    3. At claim stage, pictures of field image to be taken every 2 days from the date of event as below till field ground is seen when recedes in case of flood, inundation. Illustration provided as below
      1. 1st set of images – date of event (T day of event. T to be counted from date of event of one day after the event)
      2. 2nd set of images – T + 2 days
      3. 3rd set of images – T + 4 days
      4. 4th set of images – T + 6 days
      5. 5th image – when ground is seen when water recedes
  3. Images to be provided as per instructions in table above

  5. Note: if field image as evidence is not received at the stage of claim as per above requirements, call centre team shall make three attempts to customers over three days for field image. Failure to receive field image, claim team shall send communication letter to customer for submission of field image within 7 days.

  2. Photo Identity Proof of insured and claimant.
  3. Copy of FIR, Chargesheet and Final Report/ Panchnama / Police Inquest Report (wherever these reports are required as per the circumstance of the Accident) duly attested by the concerned Police Station.
  4. Copy of Medico-Legal Certificate (wherever it is required as per the circumstance of the Accident) duly attested by the concerned Hospital.
  5. Any other relevant document required by us for assessment of the claim.
  6. Death certificate.
  7. Postmortem Report (if conducted).
  8. Identity proof of Nominee or Original Succession Certificate/Original Legal Heir.
  9. Certificate or any other proof to the satisfaction of the Company for the purpose of a valid discharge in case nomination is not filed by deceased policy holder.

  7. Self-attested copy of all diagnostic reports (including imaging and laboratory) along with the medical prescription & copy of invoice / bill and receipt from the diagnostic centre.
  8. Self-attested copy of MLC / FIR copy – in Accidental cases only
  9. Self-attested copy of death summary & death certificate (in death claims only)
  10. Any other support requisitioned by the claims department
  11. KYC documents
  12. Note: If these details are not provided in full or are insufficient for Us to consider the request, we will request additional information or documentation in respect of that request.

    3. Attested copy of Death Certificate.
    4. Burial Certificate (wherever applicable).
    5. Attested copy of Statement of Witness, if any lodged with police authorities.
    6. Attested copy of FIR / Panchanama/ Inquest Panchanama.
    7. Attested copy of Postmortem Report (only if conducted).
    8. Attested copy of Viscera report if any (Only if Postmortem is conducted).
    9. Claim form with NEFT details of Nominee along with Cancel Cheque/ Passbook copy.
    10. Medico Legal Certificate (wherever it is required as per the circumstance of the Accident) duly attested by the concerned Hospital.
    11. Identity proof of Nominee or Original Succession Certificate/Original Legal Heir.

  2. Permanent Partial /Total Disablement cover
    1. Duly Completed Personal Accident Claim Form signed by insured.
    2. Attested copy of disability certificate from Civil Surgeon of Government.
    3. Hospital stating percentage of disability.
    4. Attested copy of FIR.
    5. All X-Ray / Investigation reports and films supporting to disablement.
    6. Claim form with NEFT details of Nominee along with Cancel Cheque/ Passbook copy..
    7. Any other relevant document required by us for assessment of the claim.
  3. Temporary Total Disablement
    1. Duly completed claim form
    2. Proof of accident
    3. Proof of disability from treating doctor
    4. Any other relevant document required by us to assess the claim.
